As it often takes some time for records to be forwarded from your former practice, all new patients are asked to complete a registration form, a health questionnaire and in some cases attend the surgery for a New Patient check with a nurse.

You will need to complete all printable forms by hand and then bring them to the surgery or email them to [email protected]
New adult patient registration form and health questionnaire (for adults 16 years and over)
New child patient registration form and health questionnaire (for children under 16 years old)
We offer new patient health checks to those patients who wish to have one but they are routinely offered to patients on more than one repeat medication or those suffering from a chronic disease. All patients over 16 years of age are offered a HIV test and a Chlamydia screening test if you are between 16-24 years old.
When you first attend the surgery you will be required to sign a GMS1 form in person to complete the registration process. This simply involves a signature and will be provided at the front desk. We also ask you to bring in photographic ID.
You will be allocated a named, accountable doctor who is responsible for co-ordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.
