We have temporarily introduced this form to allow you to request your medication via the web site. If you do not wish to use this form you must telephone your normal pharmacy.
After completing this form, all prescriptions will be sent to your preferred pharmacy usually within 2 working days but please allow 3 working days.
If we do not have a preferred pharmacy on file we will contact you by phone.
If you would like to change your preferred pharmacy please phone us first before completing this form.
Please allow 2 to 3 working days before collecting your prescription.
Remember the details you submit will be sent to us by unencrypted email via the Internet. This means that complete confidentiality is not possible because we cannot guarantee the security of the Internet. If you are not comfortable with this arrangement you may prefer to telephone your usual pharmacy and ask them to request your medication from us.
We do not take prescription requests over the telephone.
