Housing Applications and Medical Information
Stockwell Group Practice does not provide housing application letters or supporting statements requested by patients.
This includes:
- Housing priority or banding letters
- Medical reports for housing applications
- Supporting statements for rehousing
- Letters describing housing need or personal circumstances
Housing decisions
Housing decisions are made by your Local Authority or housing provider. They use their own assessment processes to determine eligibility and priority. The GP practice is not involved in housing allocation decisions.
Requests for medical information
If the Local Authority requires medical information, they must contact the practice directly with your written consent. We will provide factual medical information only.
This may include:
- Diagnoses
- Relevant medical history
- Current treatment
- Objective clinical information relating to daily function
We do not provide:
- Opinions or recommendations
- Statements of housing need or priority
- Supporting letters for individual housing applications
Patient requests
We do not accept requests from patients for housing letters or supporting statements. Appointments cannot be used for this purpose.
What you can do
- Complete your Local Authority housing assessment form
- Use the NHS App to access your medical summary
- Submit a Subject Access Request if full records are required
Summary
We only provide factual medical information when formally requested by authorised organisations with appropriate consent. We do not provide housing advocacy or support letters.
