New Patient Registration

registrationThe doctors welcome new patients who live within our practice area.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a registration form, a health questionnaire and attend the surgery for a New Patient check with a nurse.

To register with practice you will need to live within the catchment area; please check with the reception staff on 020 7274 3223 and with the map on the link found below:

Before you register we will need proof of your address (utility bill/ bank statement).

Medical treatment is available from the date of registration.  Please contact reception for further information.


HOW TO OBTAIN A MEDICAL CARD

Medical cards are issued by Primary Care Support Services NOT the Practice. For new patients registering with the Practice a medical card will automatically be sent to your registered address within 14-21 days.

For existing patients’ requiring a replacement medical card, they will need to come into the Practice and complete a ‘medical card application form’. A medical card will then be sent to the patient at the address contained on the medical database.
